Mapping |

I wanted to get this out so folks had some time to look at it. I
hope that the SDO TC will vote on my proposal to add the 4 unsigned types
on Tuesday.
C++ (note that SDO for C++ has not yet used any STL functionality,
but we already have it in ComponentContext, so using it for ENTITIES, IDREFS
and NMTOKENS seams reasonable).
C
The topic that potentially need the most consideration is mapping between
anySimpleType and void *
