I was looking at this again, and realized
the proposed API changes represented a bit too much repetition. This
draft has three key changes, SCACallback() (and the async variants) are
removed and SCAInvoke() is now used for all operation invocations. The
Async API would only have two functions so the Sync API and the Async API
have been merged into a single SCA API. Finally for greater symmetry
SCALocate() and SCALocateMultiple() have been renamed to SCAGetReference()
and SCAGetReferences() respectively.
The end result is the API is smaller,
and a bit more symmetric.

My prior draft was incomplete. The async invocation APIs were listed
in the wrong place in chapter 6 and the corresponding asynchronous callback
invocations methods were missing. I also added text describing how
async invocation works for program-bassed component implementations.
This replaces the prior draft.
