Issue 93 - Allow external attachment for intents - lastet proposal

Hi,

As discussed in the last telecon, Mike and I iterated over the Issue 93
proposal to add:
1) Support for the second half of now deleted POL40026 (delayed activation
of policy) back into the spec as it had been lost in previous revisions.
2) Iterating the <intentAttachment> capability to support both intents and
policySets, in the form of what is now called <external Attachment/>.

The changes you see in this document are ONLY a delta on top of the
previous proposal.  The full proposal for issue 93 can only be seen by
diff'ing cd02-rev6 against this newest proposal (something I will have to
do when I try to apply this beast to rev7).
