I didn't see any attachment in my copy, so I suppose the server still managed
to trim it off.

Anyhow, here're the files, with some brief explanations:

User uses this "locked" version of Excel form for data entry;  "locked" means
only those fields which expect user inputs are permitted for data entry and
otherwise not:

The filled form then gets transformed by data extractor into:

http://softml.net/jedi/biztrans/sample/XIP-TMP-20060626-223631-N2PJ.xml


If user wishes to modify any part of form, such as logo, positions, colors,
fonts, paper sizes, and more importantly, namespace values, data path
constructors (the modified XPath syntax) and so on, they would use the
"unlocked" sample version as follows:
