Extension applies to the content model as a whole; think of adding a new sequence around whatever's there, and adding new stuff to the end of that sequence. So: (a|b) becomes ((a|b), c, d, e...) I believe you can restrict choices the same way you restrict everything else -- by laboriously copying over the content model and taking out only allowable bits.
