A couple more points:
3. Shouldn't we now drop our three sub folders in the codelists folder since
we no longer distinguish use, placebo, etc? (Do we even need a 'codelist'
folder now?).
4. I'd comment that it may not be possible to avoid the use of the name
'DerivedCodeType' if we base the 'standard' codes on separate Schema modules
but do we actually need to do this now - can't we just have everything we
need for these
'standard' (i.e. Specialised DataType, xsd-validatable codes) in the sdt
Schema module?

It seems we still have a variety of options. However we seem to now have at
least one here that works.
