Revised issues list

I've taken another pass through the issues list posted by Anne
Hendry 14 July between the Atlantic and Pacific TC calls.  The new
version is attached as a "zzz" file (actually a zip file -- just
change the file extension).  Key changes:

 - Entered decisions from 15 July Pacific TC call

 - Split up the issues list into a set of spreadsheets, each one
   corresponding to one of the six categories assigned during the
   triage and confirmed in TC calls (except that category 1, "1.0
   issue", and category 2, "Possible 1.0 issue", have been
   collapsed into a single category in the first spreadsheet);
   this means that we will have to move items to different
   spreadsheets as they are disposed of, but it makes the
   categories a lot easier to work with

 - Deleted the column for Category as no longer necessary

 - Did a lot of formatting changes to try to make the
   spreadsheets a little easier to work with

N.B.: The issues list belongs to either me or Anne, depending on
which of us has it checked out.  All item changes to the
spreadsheets have to go through Anne or we'll have real problems
with issue version control.
