I could not find any code list metadata in that PDF file.

On page 79 I see metadata, but it is mis-aligned 
by one row and there is no canonical URI.  It is 
the canonical URI that I'm asking for above.

In fact I couldn't find any canonical URI strings in the entire PDF file.

Have you created any genericode files?  The 
canonical URI is a required component of list identification metadata.
