[Bug 214] Correlation ID in AssertionType should be removed

Correlation is now deferred to the addressing scheme and therefore we do not
need this parameter any more.  Also need to add text to context to describe this
parameter and its use, if we retain.  See note and figure 4 in WS-Context.  Doug
suggested making the note mention correlation and removing the word 'handled'
plus removing the parameter.  Doug proposed this and Mark seconded and it was
passed unanimously.
