In the interest of making putting forward a concrete proposal, I propose: Add after the first paragraph in section 3: "Some RM header blocks may be added to messages that happen to be targeted to the same endpoint to which those headers are to be sent, thus saving the overhead of an additional message exchange. Note that reference parameters should be considered when determining whether two EPRs are targeted to the same endpoint." The placement and original language are borrowed from Doug's original proposal. It's also possible to put it in 3.6 where "piggy-backing" seems to be defined.
