This update includes the fixes identified in issue 172:

This rev is being submitted as a .zip file, which contains the following:
- Current version of examples doc:
ws-sp-usecases-examples-draft-20-01.doc
- Sample requests and responses pulled from log files (which include
examples from successful execution of all use cases as well as one demo of
an error condition)
- a zip file containing the policies and certs used during the interop.
- a readme file with a brief description of the contents.
