[xacml] XACML XML DSig Profile 0.2

Attached is a new draft of the XACML XML DSig Profile.  I have
tried to make the Profile consistent with the SAML DSig Profile,
and have also incorporated numerous edits and improvements
suggested by various reviewers.  There is a new example showing
how an XACML policy can be included in a SAML Attribute Assertion
that is signed.
