The combiner parameters

All,

During the previous call we tried to understand what the combiner 
parameters were meant to be referring to. I searched the email archives 
and it appears that the correct interpretation is that the combiner 
parameter without a reference is associated with the combining algorithm 
itself. See here:

The spec does say anything about this though, so we should add some text 
about that.
