The parameter "level" was supposed to be in HAZARD, but got left out by editorial error from the application of the San Jose agreement on HAZARD - some how the paragraph about fomrs got in but the substance was missed.

Ah, yes I remember this and my notes confirm. OK, the suggested text is in line with what we discussed as San Jose.
