[bt-spec] Even newer BTP draft

Thanks to Alex, we now have a draft of BTP with revised XML and XML schema.
I've put this up as 0.9.1.2 - change-marks are *additional* to 0.9.1.1, so
the base is still the document as agreed after the 16 January meeting (but
with issue 82 marked by a comment not change-bars)

0.9.1.2 also has the proposed solution for the very minor issue 74, which is
up for vote at today's meeting.

Updating the schema covers issues 10, 11 and 94.  Obviously some of the
outstanding issues may affect the XML (Alex has aligned with the 0.9.1.1
changes, so 0.9.1.2 is self-consistent (intended to be, at least :-). I am
marking those as "solution proposed"

The XML description and schema update should include all the corrections
that Jim Webber has pointed out (messages sent offline to Alex and me -
thanks)

The schema section has been moved to be part of the XML section, before the
bindings, rather than after. This move is not change-marked
