[bt-spec] More on Context

Hello everyone,

In addition to the thoughts outlined in issue 103, I'd like to add that
context should be transported out-of-band (i.e. in the SOAP bindings as a
field in the SOAP envelop header).

Assuming the scenario from 103, this would mean that context is entirely
ommitted from any BTP-specific exchanges, and ends up being tacked-on only
to application messages, which seems neater.

Is it possible to tack this issue on to 103, or would it be another issue?
