[business-transaction] BTP draft 0.9.1

I have put a new draft up on the website, numbered as 0.9.1. It is just the
all-changes-accepted version of 0.9.0.4, with an editor's note at the
beginning of the section CONTEXT_REPLY & ENROL & application message ( &
PREPARED), pointing out that issue 82 has not been agreed yet. Separating
out issue 82 will be fairly complicated, and I didn't want to put the effort
in until it has been agreed the proposed solution is rejected.

Numbering as 0.9.1 was to indicate that it doesn't contain unagreed changes,
(except that it does, with issue 82). I'll make the next "convenience"
version 0.9.1.1.

Putting all the previous changes in with change-marking means there may be
some typos - missed spaces, multiple empty paragraph marks, paras flowing
together where they shouldn't. If people spot any and tell me directly, I'll
just incorporate them in the next version, change-marked but not otherwise
reported.

The pdf version has been made a different way to the previous ones, using
Adobe Distiller, not the PDF writer. This means you can use the navigation
bar to jump around the document and the table of contents act as hyperlinks.
However, it puts un-attractive and mis-aligned boxes round all the links
when viewed in Acrobat (prints ok though). Presumably there is some way to
suppress these, though I've not found out how (didn't spend that hunting) -
if anyone knows, I'd be very interested.
