Added a proposal which clarifies the upload discussion and PDP structure in Chapter 4.

Note this does not address the upload API section (currently 6.11), downloading, nor import/export (Chapter 2). It is being supplied as an incremental/partial improvement affecting both CAMP-3 and CAMP-4 as per the Chair's instruction so that obvious improvements can be made sooner.
