Updated proposal following last week's comments.

This applies the following comments from v1:

* normative reference to ZIP (Anish/Alex: http://www.pkware.com/documents/APPNOTE/APPNOTE-6.3.3.TXT )

* Roshan: add source code to the list of app content items 

* AOtto: "use camp.yaml"  (this can change back to json etc as we evolve CAMP-4)

This does NOT handle validating / signing. (c.f. AOtto's request "Some way to standardize signing/digest to ensure imterop. We can do this with a reference to OVF." in CAMP-4, and other discussion on the call.)  I'd really like to see that raised as a separate issue, unless there is a really quick easy way to resolve it better than I've done here.
