        Complete proposal for CAMP-41.

Considering the large number of detailed changes, it would be surprising if there weren't a number of errors in this proposal. However, it is probably more than 95% correct and moves the ball forward with regards to identifying and indexing the normative statements.

The main difference in this proposal is that I split the tables in Appendix C into two tables, one for the mandatory statements (SHALLs) and one for the non-mandatory statements (SHOULDs, RECOMMENDEDs, and MAYs).

I didn't try to re-factor or combine any of the duplicate normative statements (easy to spot by perusing the tables in Appendix C.)

The nature of this issue requires that, if accepted, this document shall become the next working draft of CAMP 1.1.
