Identifying tests

Hi cgmo-webcgm,

  Lofton rightfully pointed out that I didn't include an author's name
  and date in my CGM test pages. People should do so. Just a comment
  will do for now, ex:

  <!--
    Author:   yourname
    Created:  the date
    Revised:  yourname, date, comment explaining changes
  -->

  I'll modify my tests shortly.
