Response for the third question (need for a new Relationship type): 
If we agree that all of the 'associated content' entities are always to be 'dependent' on their parent object (i.e. delete of parent object cascades delete of the associated objects) then I think we do not need to expose a special relationship type.  This feels to me like we would be  dictating how the repository should model these objects, which we want to avoid.  For example we hide the (obviously required) relationship objects between documents and folders for repositories that allow multi-filing. This also simplifies the model.  I would follow the same pattern here.
