Thanks Bob, that looks a lot better. The empty page 4 with just the title is all that’s left that looks odd to me. Is there anything that can be done about that?

Sorry about the codeblocks. I restored them. As for the pagination, I inserted outputclass overrides that will force each topic to the top of the page (@outputclass="worksheet" forces a page-break both before and after a topic).

Converted markup for element names and attribute names to the DITA 1.3 XML mention elements <xmlelement> and <xmlatt>

Converted <codeblock> contents to <![CDATA[]]> sections to facilitate cut and paste from the DITA source

Removed the word "Draft" from the final row in the change log.
