Bug:
Redefining <author> in bookmap.xsd causes a schema validation error using
Xerces-C

Fix:
Move the included of the files metaDeclGrp.xsd and tblDeclGrp.xsd in
commonElementGrp,xsd and bookmapMod.xsd to the head schema documents
bookmap.xsd, concept.xsd, ditabase.xsd, glossary.xsd, map.xsp,
reference.xsd task.xsd and topic.xsd.

I moved the two included files to the head schema documents to make it
easier for users to see which files are included and also to make it easier
to redefine elements in the schemas during specialization
