I've posted the latest draft as TOCJS and HTML Help, including many comments from Paul Grosso, Erik Hennum, and Rob Frankland. I'm not posting the source quite yet, because I'm having trouble getting my keys straight. That's what happens when using DITA 1.2 constructs when DITA 1.2 development is incomplete. Until now I've been using my own processing override to resolve keys within the 1.4.2 DITA-OT, but as soon as I get the actual keys set up properly for use with DITA 1.2 I'll post the source again.
