Proposal 13092 Updated

Per discussion on 9/3, I have updated this proposal to add examples to each
of the reference entries for indexterm, index-see, and index-see-also. I
also realized that I had failed to include index-base in the list of
elements with updated content models--index-base is the base for index-see
and index-see-also and therefore must also allow ph in its content.

This does not change the nature of the proposal, only makes it completely
correct.

I have updated the proposal documents in Kavi.
