Proposal 13092: Allow ph in indexterm: Examples updated

I have updated the proposal with corrected examples using E=mc2, e.g.:

The following example illustrates using a specialization of <ph> within
<index-see>:

<indexterm>Einstein's mass and energy equation

</indexterm>
