Update to style sheets for the committee note PDF

If you build committee notes, please update the plug-in for the PDF.

I've updated the master branch at 
https://github.com/oasis-open/dita-stylesheets to include a fix for how 
Appendix titles are rendered.

The committee note PDF and HTML plug-ins work with the most recent 
versions of the DITA-OT.
