I reviewed the reference entries for learningObjectMapRef and learningGroupMapRef and both state that the default for @format *is* set to "ditamap".

So the bug is entirely in the grammars where the grammars do not reflect the intent of the spec.

Notably, the examples in the spec show explicit @format attributes, which is why we probably never noticed this bug.
