That's exactly what it's not doing.  The HTML output looks like

  <html>
  <head>
  <title>Using <b>foo</b>
  </title>
  <meta name="generator" content="DocBook XSL Stylesheets V1.16">
  </head>

So the browser displays "Using <b>foo</b>" in the title bar.
