For me it depends on the document.  Sometimes it's easier to create the
structure of a document (or part of a document) first, then put in the
text, such as for tables.  Other times it _is_ easier to tag a file
saved as "Text Only," especially if you can automate the tagging process
with an emacs macro or something.

Also, we don't always feel like printing out the original, so working
from the PDF gives me an idea how the thing is supposed to look, more or
less, particularly in regard to in-line elements, whose markup gets
erased or changed in a "Text Only" save.
