Regarding the second part of your question: I can't see, that it is possible to generate linenumbering in a document with the HTML and/or FO stylesheets. The following code creates paragraph numbering. I am not particular happy with the result (finding this kind of numbering kind of confusing) , but this is what was required in a project.
