It does not help. I tried as empty customization layer as possible:

                xmlns:exsl="http://exslt.org/common";

                exclude-result-prefixes="doc exsl set">
  <xsl:import href="/usr/local/share/xsl/xhtml/docbook.xsl"/>
  <xsl:param name="default.encoding">ISO-8859-1</xsl:param>
</xsl:stylesheet>

The only solution was to switch from xhtml to html but I'd prefer xhtml.

I used xsltproc, btw.
