This output was missing in the FO created by the 1.48 version of the
DocBook XSL style sheets, but it was present in the 1.60.1 version.

It's boldface for me as well, in PDF produced by FOP 0.20.4, on FO
created by DocBook XSL 1.60.1 (I must have blinked when I thought it
wasn't).
