Thanks. As far as AntennaHouse XSL Formatter has been concerned, this is solved: Antenna still is using MSXML v3.0, which has a problem with the "xsl:stylesheet" declaration in autoidx.xsl. (Antenna didn't throw an appropriate error, though.) Transforming the XML file to xxx.FO by using MSXML v4.0, resp. XML.NET, and then rendering that .FO file to PDF by using Antenna, solved this issue.
