The example in my book using a <?dbfo bgcolor="#EEEEEE"?>
processing instruction  for bgcolor does work.

I just tested it with the 1.62.4 stylesheets and fop 0.20.5, and XEP too.
Two of the heading cells have a grey
background when viewed in PDF.  You have
to put the processing instruction in each cell.  If it isn't working for
you, peek in the FO file to see if the table-cell in question has
a background-color property.

There is a separate bug registered against FOP 0.20.5 regarding
background-color
in table cells overwriting the cell borders.
