Actually, I had only tested it in 1.67.2 and found that it worked, so I assumed it had been a bug that was fixed. I just tested with version 1.62.0 and 1.62.4, and it works there as well. Can you look in the FO output and see if there is a fo:wrapper with a rx:key="some_indexterm" in the title block? There should also be a matching <rx:index-item merge-subsequent-page-numbers="true" link-back="true" ref-key="some_indexterm"/> in the index. If you can't upgrade the stylesheets, then you should move the indexterm out of the title.
