You are getting the logo in all three positions because you placed its reference outside of the xsl:choose statement in header.content. The choose statement is used to select which position in the header to place each kind of output. The $position values are left, center, and right. If you don't include your fo:external-graphic inside one of the xsl:when clauses, then it will be output for all positions.
