My fo file contains the following: <fo:basic-link internal-destination="cpof_copy_machine_archive">“Copy Machine and Archive Tools” (p. <fo:page-number-citation ref-id=""/>)</fo:basic-link> so it appears that the problem is occuring in the docbook modular stylesheet processing.
