    There is new namespace cf introduced in chunk templates an it
becomes present in output as
<html xmlns:cf="http://docbook.sourceforge.net/xmlns/chunkfast/1.0";>

    I guess it is safe to put this cf into exclude-result-prefixes
attribute of root <xsl:stylesheet> =)
