There are many potential causes. You need to provide some more information
to make it possible for other people to (try to) reproduce the errors. A
minimal but complete sample source document is often a very important clue.
It could also be useful to know what XSLT processor you use and what
parameters you tried. And when you say documentation, what do you mean? Have
