Strange, this problem was reported earlier this year in #1212159 and fixed.
Are you using the chunkfast.xsl stylesheet, or just setting chunk.fast to 1?
That was a mistake in my book.  Here is the errata:

a.. In Chapter 6, HTML output options, section titled "Fast chunking", the
description of how to use fast chunking is not correct. Instead of just
setting the chunk.fast parameter to 1, you must use the chunkfast.xsl
stylesheet instead of the chunk.xsl stylesheet to process your document.
