I don't know if it's a limitation of the PDF format itself, or simply of the Reader. But the vast majority of people use Adobe's Reader to view PDFs anyway, so the distinction might not be relevant.
 
The following Adobe tech note seems to indicate that the problem is fixed in Acrobat 8.0 when you browse using Firefox. It address opening specific pages, but I'm guessing that named bookmarks work the same way.

Thanks. Does that mean it’s a limitation of the Adobe Reader and its plugin? If so, does anyone know if Adobe plans to address this problem anytime soon?

Named destinations only work in PDF files that are served from Web servers, not in local copies that exist on your hard disk. This is a long-standing limitation of PDF.

With the release of FOP 0.94, FOP now supports named destinations. But I can’t get olinks from one local PDF to another to work. When I click the link, it opens the PDF to the first page instead of going directly to the fragment identifier (e.g., filename.pdf#section).

 

Is this a bug in the link that’s generated by FOP?
