Please Help: No TOC after moving to stylesheets version 1.75

I posted this before but didn't get a reply. I'd appreciate any clues.  
Works fine with 1.71.1 with the following customizations. Now I get an 
empty TOC with version 1.75.1. This happens only in the HTML output. 
Works fine with PDF.
