Hi Scott,
That example needs to be updated for the 1.75.2 version of the stylesheets.  In 
1.75.2, the stylesheets added full support for writing mode right-to-left, so that 
references to 'left' and 'right' had to be made into variables.  The 
'margin.left.outer' and 'margin.left.inner' variables were internal to the 
fo/pagesetup.xsl stylesheet module, and were made obsolete in 1.75.2  See how 
page-masters are set up in fo/pagesetup.xsl in 1.75.2 to get the new ways to set the 
left and right margins.

The region.side.extent parameter is new to that example, and that should have been 
noted in the message. The stock stylesheets don't even have fo:region-start or 
fo:region-end defined, so there are no params to set their sizes.  I added that param 
in parallel to the region-before and region-after params.
