Yes, right now this is quite hard to accomplish in the Author page. It 
is possible but hard to work with. You could position the caret right 
before the <tbody>, press ENTER to insert the <thead> and then copy the 
first row from the <tbody> to the <thread>.

How would you see an easier way to do this in Oxygen? Maybe select the 
first row, right click and have a custom action like "Make Header Row"? 
Something like this?

Indeed read-only scenarios need first to be duplicated. In Oxygen 14.1 
the "Edit" button will be active and will guide you into duplicating the 
scenario, which is a more useful approach.

The problem is that even at the end of the list item you can still 
insert other elements and ENTER shows you the list of available elements 
+ the top-level SPLIT entry.
Probably indeed Ctrl-ENTER would be an interesting way to split easily, 
but you are still pressing two keys :)

Yes, because you are inside a section and the toolbar actions you are 
probably using to insert the section understands that you want to create 
another section inside the current one.
If you move the caret by pressing the DOWN arrow you should be able to 
move inside the parent section and insert the section there.

Indeed having such promote-demote section operations would be very 
useful for Docbook. I added an improvement request for this.
