I have had some success with Serna and choosing the 4.5 pre-set, and specifying a 5.0 schema (for example: /usr/share/xml/docbook/schema/xsd/5.0/docbook.xsd).

The problem I had was my docbook xml uses XInclude which at the time of testing (+1 year ago?) Serna didn't support. An identical experience is reported on this page:
