Regarding browser performance -- I haven't studied patch very carefully but if understand correctly it still puts ToC into each HTML page. This can make pages really large. Another possible approach is to store ToC into a separate file and load it dynamically -- either as a script or using AJAX. This can significantly decrease loading time, but parsing ToC from some JS structure will take some time for larger documents.
