Here are the files. They are basically the code you sent. I tried it on my system with my "tuned" stylesheets and also with the standard Docbook stylesheets.

 

Docbook XSL 1.78.1

XML Processor: XSLTPROC 2.9.1

FO Processor: Apache FOP 1.1

 

Included is the PDF that this produced. This may be a Maven issue and not a Docbook issue.

 

Is Maven a requirement in your process or are you just trying to write a Docbook based book and used Maven as your tool as a quick test?

 

What is the big picture here?

I use MathML for all of my equations. See Bob Stayton's book for a better explanation of how to integrate MathML equations.
