To go from DocBook to MD, I do something like:
pandoc -f docbook -t markdown_github 2.xml -o 2.md
and back is:
pandoc -s -t docbook -f markdown_github 2.md -o 2-out.xml
warning: I have not used this in a few years, so you mileage may vary.

I have considered using a utility like Pandoc as a final processing step. So the chain would be something like DocBook > XSLT > HTML > Pandoc > markdown.
I have also wondered whether anyone has built DocBook XSLT that directly converts the XML to markdown.
Do you have something that works well for you?
